Insurance Investigations

Probe Services works with many SIU units and claims departments to investigate insurance fraud. Fraud in the insurance industry is wide spread and usually consists of false or exaggerated claims.

We specialize in surveillance investigations, which are designed to fit the investigation needs of our clients, in the insurance industry. Our staff is well versed with the New York State statute in relation to evidence and discovery.

Surveillance is conducted by one investigator equipped with an 8 mm/digital camcorder, covert video and audio equipment. Two investigator surveillance with radio equipment is used when deemed necessary and only with prior approval.

Surveillance is conducted on subjects to document work status, daily activities, degree of disability, mobility and overall general health. Activities observed are video documented to provide to the client a clear and detailed description of the subject's activities.

Detailed narrative reports are provided for each investigation and are designed for court presentation. Our investigators are also available for any follow up investigations and testimony if necessary.

Surveillance investigations can be conducted on an hourly or daily basis. We also offer flat rate pricing for blocks of 4, 8, 16 and 24 hours. This is an all inclusive rate which includes surveillance time, travel, mileage, record searches, clerical costs, audio/video presentation time and consultations.

Workers Compensation

By completing interviews with claimant's employers and witnesses, we provide our client with detailed information pertaining to the actual claim. This includes an in-person claimant interview which outlines all details regarding accident, injuries sustained, notification and subsequent medial treatment. The claimant's health history is also reviewed for potential 15-8 issues. We can obtain signed or recorded statements, medical authorization forms and any other documents necessary to facilitate the claim.
Personal interviews are also conducted with the employers detailing all known facts of the accident, including injuries sustained and notification. The claimant's personnel and medical records are reviewed for 15-8 possibilities. The claimant's supervisor and any witnesses can be interviewed on all controverted claims.

Probe Services will conduct a comprehensive study of the workers compensation board file at the New York State Workers Compensation Board, throughout upstate New York. Copies are secured of all essential paperwork regarding the file. We can also collect medical and/or employment records that relate to a claim.

Our office also conducts annual/bi-annual activity check investigations with claimant's that receive long term or pension benefits. This includes an in-person direct interview with the claimant and the gathering of information from the neighbors if requested. We would verify the claimant's address, employment status, expectations on returning to work, daily activities, current disability/health status, the names of medical providers and frequency as well as verifying the benefits are being received on a timely basis. Activity check investigations are done on a flat rate basis with all inclusive pricing.

Also available is a covert activity check investigation in which information is gathered from neighbors and/or sources from the claimant's community. This can be used as a tool to detect insurance fraud and assisting a claims representative to determine whether or not additional investigation/surveillance is warranted. Information gathered is to verify the residence of a claimant, document his employment status and/or daily activities. These investigations are also available with flat rate pricing.
